Your child's doctor can either fill out this attached form or send us a sports clearance letter from their office.
FAQ
Do I need to fill out the Rank One forms every year?
YES. The Rank One forms need to be completed every school year. They reset at the completion of the academic calendar.
Does my child need a physical every year?
NO. Physicals are good for TWO calendar years. Every two years you will need to send an updated physical.
Is the Rank One concussion form the same as the concussion test?
NO. The Rank One concussion form is separate from the concussion (impact) testing. Impact testing is only for certain sports / grades. Your coach will inform their athletes if they need to get impact testing done. The concussion form needs to be signed off every year.
Is everything online?
YES. Nothing needs to be delivered to school. The Rank One forms are online and the physical can be emailed or faxed. A photo will do.
What is the fax number?
Due to my office being at Kennebunk High School, all SPORTS FORMS can be faxed to KHS Fax # (207) 985-1355. All other MSK forms would be faxed to MSK Fax # (207) 467-9059.
