Registration to participate in MSK sports:
In order to be eligible for MSK fall sports, students MUST have completed their Rank One registration AND be marked "Eligible to Participate" before their first practice session. Here is the link to the Rank One Parent Portal.
To complete registration:
*1) Show proof of a current sports physical (good for 2 years) -- PLEASE have the Physician FAX the completed physical form to MSK at FAX #(207) 467-9059 (You can use our physical form (MSK 2024-2025 Medical Release Card ) or have the physician send us their own form)
2) A signed online Athletic Card, and
3) A signed online Concussion Management form
*Students who participated in a sport last year do NOT need to have a new sports physical UNLESS it has expired--check Rank One if you're not sure!
PARENTS--Regarding student travel for AWAY EVENTS:
a) HANDOUT regarding Sports Travel (please READ to find out when a form is needed)
b) MSK Travel Release Form 2024-25 (should be submitted 24 hours in advance)
