KHS/MSK RANK ONE INFORMATION FOR ATHLETICS
Please see below the information for completing the RANK ONE online forms for the upcoming school year. There is also information listed regarding physical information. All online forms and physical information must be completed before the start of the Fall sports season.

On-line Information and Physical Information for Athletics
All athletic forms will now be completed online and need to be completed by August 15th for KHS fall sports. The instructions are below:
All Kennebunk High School athletes (9th, 10th 11th and 12th graders) and Middle School athletes (6th, 7th, and 8th graders) will need to complete the forms below.
You will need the following information before you start:
Your student’s 5 digit Power School ID
Date/Month of the student’s last Tetanus (TD) booster
Your Insurance Company, Policy Number and Name on Policy

First: go to Download and Print.
All incoming 9th graders will need an updated physical. The Medical Release Form is all you need to get signed when you get your child’s physical. If your athlete has had a physical after January 1, 2021, please have your physician sign off on this form or have a signed note from the physician’s office stating that the student is cleared to participate in athletics.
Physicals are valid for 2 years from the date of the exam. If your child’s physical is going to expire prior to or during the 2022-2023 school year, please send an updated release after the physical.

2. Second: go to Electronic Participation Form
Fill in all areas on the Athletic Card.
Sign off indicating that you and your student have read the Athletic Handbook and all of the Concussions information.
Go directly to the Concussion Management page. Please read all of the information and then sign off at the bottom.
You have now completed all online forms and will receive an email from Rank One.
You will need to deliver their physical form to the athletic office or fax it to 207-985-1355.

Protocol for Away Games
Protocol for away games is as follows:
Athletes will get on the bus for away games in the front of our building between 2:15 and 2:30. Athletes should wait for their coach before they get on the bus.
Athletes are expected to travel to and from the games by RSU 21 buses.
In the event that a parent or guardian would like to take their son/daughter home from the away contest, the parent needs to sign their child out with the coach. If a parent wants their child to go home with another parent, they will need to get that in writing to Mr. Noel for approval. There is a form on our website that can be completed and brought into school prior to the contest or emailed to Mr. Noel at knoel@rsu21.net
